Requesting Medical files
REQUESTING YOUR MEDICAL RECORDS
According to the "Patients' Rights" law, as a patient, you are entitled—except in certain cases*—to obtain a copy of your medical record.
Laboratory and imaging results are directly accessible via our patient portal.
To request your medical record, please click on the Online Request Form link.
If you encounter any issues or have questions, you can contact the Medical Records Department by email at: dossiersmedicaux@europehospitals.be or by phone at 02/614.24.62.
This right may also be exercised by a third party acting on behalf of the patient: either a "trusted person" officially designated for this purpose, or the patient’s legal representative if the patient is a minor and/or unable to exercise their rights independently.
For minors, a copy of both the child’s ID and the requesting parent’s or legal guardian’s ID is required, along with proof of parentage if not stated on the child’s ID card.
Whenever possible, the medical record is sent electronically (via secure email). A printed version can be provided upon request.
Printed records can be collected by appointment only on Mondays, Tuesdays, Thursdays, and Fridays between 9 a.m. and 11 a.m., and no later than 15 working days after written confirmation of the request.
Documents are sent via secure email as promptly as possible.
Postal delivery within Belgium is exceptional and will only be done via registered mail with acknowledgment of receipt. In this case, the non-profit organization Cliniques de l’Europe cannot be held responsible for any potential loss of documents.
If the patient is deceased, no copy of the medical record may be issued or sent if the patient did not have a legal representative during their lifetime.
Following the patient’s death, access to the record is only allowed through a physician designated by the deceased's relatives, subject to a substantiated request.
Your request will therefore be forwarded to the Legal Department, who will follow up with you directly.
*Article 9 § 3 of the law of 22 august 2002 : A professional practitioner may refuse to issue a copy of the medical records if he/she has clear indications that the patient is under pressure to hand over a copy of their file to third parties.
