The letter needs to be completed by your general practitioner or referring doctor and sent by email to our secretariat (Sint-Elisabeth: secretariat.nephro.se@europehospitals.be; Sint-Michiel: secretariat.nephro.sm@europehospitals.be). This will enable us to provide you with the best possible assistance.
